The enrollment process is as follows:

Download:
Download the set of enrollment forms you need by clicking in the appropriate icon in the next column.

Print:
Print these forms, fill out the enrollment pages, include the appropriate fees, and mail or bring these to our offices.

Mail:
You cannot enroll online. We require original forms with original signatures. You can mail these to:

If you are currently enrolled and are in need of additional forms (Report forms, Curriculum forms, etc.), you can find those copies ....[ here ].

The enrollment forms have been saved as Acrobat Reader© files.

If you do not have the Acrobat Reader software, you can download a free copy from the Adobe Systems website at adobe.com.

Adobe Acrobat files (or pdf files) are small and easy to access online. They print well and should be easily handled by your personal printer.
