Here you will find single copies of required forms for the current school year. You cannot fill them online, but instead need to download and print these to be filled, signed, and mailed back to our offices at:

Adobe Acrobat files (or pdf files) are small and easy to access online. They print well and should be easily handled by your personal printer.

Parents are required to maintain an attendance report and a grades report and submit these to the school office at the end of each semester.

The first semester reports are due on or before January 10th, the second semester reports are due on or before July 10th.

If a student transfers out during the middle of the semester, these reports must be submitted immediately.

A list of the courses the student will be covering during the school year needs to be submitted no later than 30 days after enrollment.

Request Form:

This form, signed by the parent or legal guardian, must be submitted when requesting a student's Driver's Permit, High School Transcript, an Employment Form, and other requests.

When submitting for a Driver's Permit, the student's file needs to be up-to-date (all due semester reports in, all fees paid, curriculum list on file).

Also, the student needs to be enrolled a minimum of 30 days with Aaron Academy and have a copy of his/her birth certificate in our files.
