Parents are responsible to review, sign, and submit the Consent to Participate documents and the Sports Physical listed below in order for their student to be eligible to participate in our athletic teams (workouts, camps, practices, games, tournaments, etc.). No exemptions.

Please note that the Consent documents can either be printed or submitted online, while the Sports Physical form can only be printed. (Some doctors request a blank form from us, others use their own form).
