| 1 | The primary teacher is required to maintain these two reports for each student and submit them by the due dates for both semesters of the school year. |
| 2 | The First Semester report covers school work between July 1st through December 31st. |
| 3 | The Second Semester report covers school work between January 1st through June 30th. |
| 4 | If a student transfers out during the middle of the semester, these reports must be submitted immediately to allow our office to update the student's file. |
